A New York State DOH/OMH/OASAS program for eligible Medicaid members, geared for individuals with medical, emotional, or substance use challenges. Through this program, eligible individuals will be assigned a Care Manager, who serves as their personal advocate and guide through their difficult journey.
As the program name suggests, it integrates the care from different dimensions: doctors, therapists, psychiatrists, social services benefits, etc., all under one virtual roof, creating one comprehensive plan with one goal of providing hope and concrete solutions.
Eligible high-need clients who qualify for Medicaid HARP (Health and Recovery Plan) can receive HCBS/CORE services, such as rehabilitation, peer support, prevocational training, psychosocial rehabilitation, and the like. That’s a personal coach for several hours a week.
These Community Oriented Recovery and Empowerment (CORE) Services (AKA Home and Community-Based Services) are meant to keep individuals integrated within the community and reduce hospitalizations.
Integrated Health is the latest initiative at Hamaspik of Rockland. Our goal is to ensure access to appropriate services, improve health outcomes, reduce preventable hospitalizations and emergency room visits, track and monitor patient outcomes, and avoid unnecessary care.

support for the OPWDD population. Hamaspik of Rockland County is part of the New York State
Hamaspik Association—a membership association comprising of 8 human services agencies providing services in 25 counties. Comfort Health is a sister-company to Integrated Health offering
similar services to those under 21
While business as usual would mean helping clients with doctor’s appointments, food security programs, medical transportation, and the like. Here’s a sampling of the magic going on at Integrated Health: Arranging volunteer live musicians to cheer up a homebound patient with a debilitating medical condition; seeking housing options for a young man being discharged from a psych ward; helping an overwhelmed young mother cope; assisting an older woman reconcile with her estranged adult children; introducing a family to several community food-security programs; and working with various cancer-support non-profits to help a patient obtain financial and other assistance.
Other distances our dedicated staff have gone include: visiting a woman with emotional instability at her home to simply hear her poetry; enjoying the personal growth-conducive setting of the natural outdoors with a young adult while sharing practical day structuring tools; negotiating with a client’s irate landlord; arranging medical transportation; and setting up social groups for a lonely homebound person.
That’s what we call going above and beyond the call of duty.
